The information below will help make your visit easier and more effective.

Financial Policy & Insurance

Patients are responsible for payment at time of service. Please remember that your insurance policy is a contract between you and the insurance provider; therefore, the clinic cannot guarantee payment of your claim and must consider patients responsible for any services not covered by their insurance.

Digital Registration

There are several options for patient registration. You may receive an email to complete your registration prior to your appointment or you may complete the registration at the time of check-in. 

Patient Forms are also available to download if you prefer paper. 

What you should bring to your appointment

In order for your physician to better serve you and provide the best possible care, please bring the following items to your appointment:

  • If you have insurance, please bring your Health Plan Insurance card
  • Any necessary or related medical records
  • A list of the medications you are taking, including dosage and the way you are taking them
  • A referral from your primary care physician if your insurance requires it
  • Any related x-rays, ultrasounds, CT scans or test results

Post Appointment

After your appointment, you may login to your myMANA account to view your health record and test results. Most information from your visit will be available within 4 business days, but some test results may take longer.
